Washington post-acute practice group responds to rising demand for its services

Swenson Healthcare, a clinical practice group dedicated to the care of patients in post-acute care facilities, announced today the addition of two senior managers to newly created positions on the executive team, effective immediately.

Sara Walden RN joins the group as Chief Nursing Officer. Sara has deep experience in geriatric nursing and administration. She has spent the past six years with Life Care Centers of America in increasingly senior positions, most recently as Director of Nursing at Life Care Center in Port Orchard, WA. In this newly created position, Sara oversees development of clinical programs designed to measure and improve the quality of patient care in the facilities served by the group’s clinical team. Under Sara’s supervision, Swenson Healthcare will be initiating new clinical initiatives in areas such as antibiotic stewardship, antipsychotic reduction, opioid reduction and fall prevention, designing methods and procedures to be scaled and deployed to all Swenson Healthcare facility clients in Washington state.

Dahan Pham, CPA, MBA, CMA has been selected as Chief Financial Officer. Dahan brings over 18 years of management experience in accounting, finance, information systems and operations in a variety of industries. In her new position, Dahan will focus on bolstering the financial and reporting infrastructure with the goal of supporting and promoting the effective delivery of patient care from the group’s clinical staff. Dahan will provide leadership in areas such as forecasting, budgeting, process improvement, reporting and analysis. Her prior senior management experience includes Kiel Mortgage, SEFNCO Communications and Cascadia International Corp.
